Food phosphorus comes in two forms: natural (absorbed ~40-60%) and inorganic phosphate additives in processed foods (absorbed ~90-100%). In CKD stages 3b-5 and dialysis, dietary phosphorus targets are 800-1,000 mg/day. Natural high-phosphorus foods include nuts, seeds, cheese, and legumes; processed foods with 'phos' on the label (phosphoric acid, sodium phosphate) are the bigger trap. Phosphate binders taken with meals reduce absorption.
