Rhabdomyolysis is the breakdown of muscle tissue that floods the blood with myoglobin — a pigment that clogs and damages kidney tubules. It follows crush injuries, extreme exertion, seizures, certain drugs (statins, antipsychotics), and alcohol. The signature warning is dark cola-colored urine; aggressive IV fluids, started early, prevent most kidney injury.
When muscle breaks down, myoglobin pours into the blood. The kidneys filter it — and myoglobin is directly toxic to tubule cells and forms casts that block them. Combined with the fluid shifts that leave victims dehydrated, it produces the classic 'crush kidney' injury.
Common triggers: crush and crush-compression injuries, being found down after falls or overdose (immobility), marathon-style exertion and eccentric exercise, seizures, heatstroke, alcohol binges, and — rarely — statins (usually with interacting drugs or kidney disease).
The tell-tale sign is dark, cola- or tea-colored urine — from myoglobin. Muscle pain, swelling, and weakness follow, sometimes with fever. Not everyone notices the urine; a CK blood test is definitive (often >5,000-10,000 U/L), with creatinine rising 1-3 days later if kidneys are injured.
The highest kidney risk sits at the intersection of three factors: high CK, dehydration, and acidic urine — the 'triple threat' that aggressive fluids target.
The treatment is fluids — large volumes of IV fluids (often with bicarbonate) started immediately, flushing myoglobin before it clogs tubules and keeping urine flow high. This single intervention prevents most AKI.
If AKI still develops: standard AKI care, with dialysis for severe cases. Most rhabdo-related AKI recovers — the kidney tubular damage regenerates — but a minority need long-term dialysis, and recovery can take weeks. Statin-related rhabdo means stopping the drug and checking muscle enzymes.
