As eGFR falls below ~30, symptoms accumulate: fatigue and anemia-related tiredness, swelling from fluid retention, itching from retained toxins, poor appetite and nausea, and sleep problems. These overlap heavily with other conditions, which is why labs — not symptoms alone — define the stage.
Fluid overload shows as swollen ankles, weight gain, and shortness of breath on exertion. When it progresses to breathing difficulty at rest or confusion, it becomes urgent.
Shortness of breath (especially lying flat), chest pain, confusion or altered mental state, very low urine output, and uncontrollable nausea are emergency signals — they indicate fluid overload, acidosis, or uremia at dangerous levels.
