Heart failure and kidneys are a two-way street: a failing heart starves the kidneys of blood flow (prerenal AKI), and congestion backs pressure up into the kidneys, directly damaging them. Managing the balance — decongest with diuretics without crashing kidney perfusion — is the art of cardiorenal care, and SGLT2 inhibitors have shifted the odds favorably.
When the heart pumps poorly, the kidneys see low blood flow (prerenal). But the bigger modern insight: venous congestion — high pressure in the veins draining the kidneys — directly injures them, driving tubule damage and reducing filtration even when blood pressure is adequate.
The kidney then worsens heart failure: fluid retention, RAAS activation, and worsening anemia create the vicious 'cardiorenal syndrome' loop that drives hospital readmissions.
Diuretics are the mainstay of congestion — and they commonly raise creatinine as fluid leaves (hemoconcentration) and kidney perfusion transiently dips. Distinguishing 'effective decongestion with a creatinine blip' from true injury is the cardiorenal skill: falling BNP, rising urine output, and stable or improving clinical status favor the former.
The red flags: rising creatinine WITH worsening congestion, hypotension, or hyponatremia — that combination demands a medication review, not just a lab glance.
The foundation (GDMT) — ACEi/ARB or ARNI, beta-blockers, mineralocorticoid antagonists — protects both organs; small early creatinine rises on RAAS blockade are accepted and usually settle. SGLT2 inhibitors (empagliflozin, dapagliflozin, sotagliflozin where approved) now sit in the foundation: they cut heart-failure hospitalizations and slow CKD progression simultaneously.
Avoid the insults: NSAIDs, contrast without hydration, and volume depletion. Loop diuretic doses are tuned to weight and urine output, and patients track daily weight — the cheapest heart-kidney monitor.
