Pre-dialysis CKD keeps protein moderate (~0.8 g/kg/day) because excess nitrogen load isn't helping and may accelerate progression. Once dialysis starts, the math flips: dialysis strips amino acids, so protein needs rise to ~1.2-1.3 g/kg/day.
The practical tension is phosphorus — the best proteins for dialysis (egg whites, tofu, fresh fish) are low-phosphorus, while red meat and organ meats deliver protein with a heavy phosphorus load. Hit the target with the right sources.
Moderate protein is the pre-dialysis target — excess protein is not helpful at this stage and higher intake may accelerate progression in some patients.
Dialysis removes amino acids, so protein needs rise. Choose low-phosphorus sources and spread intake across meals.
